What to Bring to Your First Appointment

Below are the forms you'll need to start on the road to wellness with FoodWise Nutrition!  Please print them out, fill them out, and bring them with you to your first appointment. 
It helps if the forms are as complete as possible.

Please fill out both your welcome packet and registration form!

Please bring:

• Your Welcome Packet: download now in PDF format or MS Word format
• Your Registration Forms: download now in PDF format or MS Word format
• Your Insurance card (if applicable)

If you have them, please also bring:

• Lab or blood work drawn in the last year
• Diet diary
• Any other food or medical information you deem pertinent

You are responsible for any portion of your medical bill your insurance provider does not pay.

At this time, FoodWise does not accept Medicare.

United Healthcare requires payment at time of visit.
You are responsible for obtaining reimbursement.
